Case Manager / Case Manager Nursing with Quality Responsibility, Nursing Specialist HF, Workload: 80% - 100%

We are looking for a dedicated Nursing Specialist HF with case and quality responsibility, starting immediately or by agreement. You will be our point of contact for staff on the tour, take responsibility for the care processes, and actively shape quality assurance.

Your tasks

• Case responsibility & care coordination: Managing the care processes for our clients, determining service needs, creating care and support plans, ensuring professional care

• Quality assurance: Implementing QM measures, controlling, audits, and supporting staff in complex care situations

• Leadership & consulting: Professional leadership and point of contact for staff, support in questions or emergencies, training and guidance of colleagues

• Care interventions: Performing nursing interventions for clients, interventions, and electronic documentation

Your profile

• Completed basic training as a Nursing Specialist HF

• Several years of professional experience in nursing, ideally with case responsibility

• Independent, responsible, and team-oriented working style

• Analytical thinking and quick comprehension

• Further training in the quality segment is an advantage

• Good IT user skills, experience with SCODI process management tool is an advantage

• Good German language skills, both spoken and written

• Enjoyment of administrative and organizational tasks
